Sunderland Weight Management Programme

If you are looking for help to lose weight and make longer term lifestyle choices, the new Sunderland Weight Management Programme may be for you.

It's free and available to anyone over 18 who is Sunderland resident or registered with a Sunderland GP and has a qualifying body mass index.

You will receive:

  • 12 weeks of tailored help and support from our wellbeing experts
  • Meet likeminded people who can help you stay on track and reach your goals
  • Weekly group sessions at venues across the city
  • Tailored physical activity sessions
  • Support with healthy eating
  • Help to make long term lifestyle changes
  • Tips to help your mental health with mindfulness techniques

How do I get involved?

To qualify for the programme, you must meet all of the following criteria:

●       Be a Sunderland resident or registered with a Sunderland GP if you live outside the local authority boundary

●       Aged 18+

●       Have a qualifying Body Mass Index of 30+ (27.5+ for BAME).

You can use the NHS BMI calculator to check your body mass index and find out if you are a healthy weight.

Please note this programme is not suitable for pregnant women. You can find out more about keeping active and maintaining your health during pregnancy at Keeping well in pregnancy - NHS (www.nhs.uk)

To register;

You can self-refer into the programme at any time by completing this form.

You can also request a referral from your GP or another healthcare professional, such as your practice nurse.

Once registered for the programme, you will begin with a one-on-one consultation, which will be repeated at the end of the 12 week course, with a follow-up session at 26 weeks.
