Membership Scheme

Life will replace all other sport leisure cards, including Active 8, Active 8 plus and Wellness from 1 April 2008.  To receive your Life card simply complete the application form ( pdf 31.29kb) and take into your nearest leisure or Wellness Centre.  Life cards will become valid from 1 April 2008, but you are encouraged to apply for a card from 1 March 2008 to ensure you are ready to enjoy all the Life cardholder benefits from day one.  Benefits at each Sport and Leisure facility include;

• Use of your card as a deposit for equipment hire
• One credit booking in any 14 day period
• Two pre paid booking in any 14 day period
• Free spectating at any of the leisure centres
• A whole year to get active

To receive concessionary prices, please bring proof of entitlement to one or more of the following when you apply for your Life card:

• Income Support
• Job Seekers Allowance
• Working Tax Credit
• Incapacity Benefit
• Severe Disablement Allowance
• Council Tax Benefit
• Housing Benefit
• Carers Allowance
• Guarantee Pension Credits
• Disabled Person's Tax Credit

Others who are entitled to concessionary prices include all 'looked after children', all young people up to their 18th birthday who's parents are in receipt of the above benefits, all full time college and university students and individuals seeking asylum.
