Sunderland City Council provides a range of services intended to help older
people recover
from an illness or trauma, to regain their confidence and skills to live as
independently as possible.
The Intermediate Care Service is provided by Adult Services in partnership with City Hospitals Sunderland, Sunderland Teaching Primary Care Trust and South of Tyne and Wear Mental Health Trust. The aim of the Intermediate Care Service is to put older people and their carers at the heart of the rehabilitation process, which will maximise both their independence and quality of life. Intermediate Care Services include:
*Farmbrough Court is a residential facility for older people who have received care in a hospital or at home due to illness or trauma. The centre provides short term 24-hour care services to promote recovery and offers three elements of care: Convalescence
Rehabilitation
Dementia Care and Rehabilitation
Each element of the service is supported by a team of Reablement Assistants who work together with professional members of staff including a Practice Nurse and Care Managers. A 24/7 Rapid Response Team supports people in their own home providing short term nursing interventions and care to prevent admissions to hospital or to facilitate hospital discharges. The Rapid Response team can also refer older people into the Intermediate Care Centre. Adult Services will also assess individual needs in order to provide any necessary care arrangements prior to admission or on discharge from the Intermediate Care Service. When an enquiry is received in respect of Intermediate Care, the Council will refer to the appropriate services for an assessment.
