How to get an Assessment

Before Adult Services can arrange for a service to be provided to you, you have to be assessed as needing the service.

To have an assessment you or someone on your behalf must contact a Adult Services office and arrange for a member of staff to come and see you. Alternatively you can e-mail us.

Before anyone comes to see you, you will be sent or given a copy of  What is an assessment? (pdf document 273kb), which is a leaflet that explains what an assessment is, what to expect during an assessment and how to prepare for an assessment.

An assessment is a way of looking at your needs with you and your carers and deciding how best we can support and help you.
