The Specialist Social Care Team provides services for people affected by advanced progressive disease.  The care that can be provided includes support to patients, families, carers and significant others.  The team works closely with health and social care colleagues to provide end of life care and support to the bereaved.

The Palliative Care Team offers a confidential and sensitive service. A telephone call is all it takes to speak to one of the Care Managers (Social Workers).

How can the Palliative Care Team help you?
  • By being someone who can listen
  • Providing information on the range of services which may be of assistance to you
  • A prompt response to your needs
  • Advice and help regarding DSS benefits and financial problems
  • Access to the full range of services available from the Local Authority, for example Home Support, Housing, Residential Care, Short Break Care
  • Advice on contacting other members of the wider Palliative Care Team
  • Support for carers (relatives or friends looking after someone with a long-term illness or disability)

How to contact a Care Manager from the Palliative Care Team

The Care Managers will usually respond to requests from anyone for information or help within one working day.

Direct Payments allows Adult Services and Children's Services to make financial support payments (known as Direct Payments) to people to enable them to organise and purchase their own community care services that they have been assessed as needing, instead of receiving services we arrange for them.
